Tadalafil (trade name "Cialis" or "Adcirca") is a PDE5 inhibitor used to treat
erectile dysfunction, benign prostatic hypertrophy, and pulmonary hypertension.
Tadalafil works by relaxing the vascular muscles and increasing blood flow to
the cavernous body. Tadalafil acts by inhibiting the activity of CGMP-specific
phosphodiesterase type 5 (PDE5). PDE5 degrades cGMP in the cavernous body that
surrounds the penis. Therefore, tadalafil causes an increase in cGMP
concentration, which further causes smooth muscle relaxation and increased
blood flow to the cavernous body. Some clinical studies have also shown that
tadalafil can improve endothelial function in men at increased cardiovascular
risk and reduce urinary tract symptoms secondary to benign prostatic
hyperplasia.
